Care Coordination Nurse Specialist

POSITION SUMMARY:
Accountable for overseeing, organizing, and integrating all Care Coordination services within the CHRISTUS St. Vincent Health System.
Delivers Care Coordination services to patients based on individualized acuity and manages patient care in both hospital and clinic environments.
Fosters a collaborative and responsible working relationship with fellow Care Coordination system staff to achieve the objective of providing consistently high-quality services to patients.
Encourages optimal patient-centered care that empowers individuals, honors personal choices, and addresses the healthcare needs of patients.Collaborates closely with clinical leaders in the formulation and execution of protocols.
May operate in a traditional clinic environment and/or in a training setting (residency program) or other designated areas as required.
MINIMUM QUALIFICATIONS:
EDUCATION:
Registered Nurse, BSN in Nursing is preferred.
CERTIFICATION/LICENSES:
Current New Mexico Nursing License
SKILLS:
Extensive knowledge of healthcare facilities, policies, and issues, along with a comprehensive understanding of community patient care services.
Strong grasp of healthcare and social management principles.
Capability to coordinate various functions and activities for optimal cooperation and integration of service providers and patients within a clinic for a continuum of care services.
Ability to guide, instruct, and advise staff while effectively addressing day-to-day challenges.
Proficient in utilizing strong communication skills, both written and verbal, and demonstrating an interactive approach to care coordination.
Understanding and implementation of sensitivity and cultural care measures suitable for a diverse population.
Ability to collaborate with various levels of the CHRISTUS organization, including clinical, financial, administrative, and medical personnel.
EXPERIENCE:
A minimum of two years of experience in a healthcare setting with relevant experience in quality, care coordination, and population health is preferred.
NATURE OF SUPERVISION:
- Reports to: Manager of Population Health, Director of Ambulatory Patient Care Services

PHYSICAL REQUIREMENTS:
Ability to navigate various care settings.
Requires close work, good vision, and dexterity for writing as well as using a computer keyboard.
Ability to manage stressful situations. Must demonstrate excellent customer service skills.
