Licensed Practical Nurse Team Leader

The primary responsibility of the Licensed Practical Nurse (LPN) Team Leader is to engage in and supervise the delivery of resident care as directed by the attending physician, in accordance with established standards of care and within the boundaries of LPN training and licensure. This role may involve providing direct care to residents and overseeing the Certified Nursing Assistants to ensure optimal well-being for each resident.
Qualifications:
- Completion of an accredited Nursing Program and possession of a current and valid license in the relevant State.
- Proficiency in reading, writing, and speaking the English language.
- Current CPR certification.
- Monitor, assess, and report on resident conditions/changes, documenting in line with Festus Manor's Policies & Procedures and applicable state/federal regulations.
- Assist in gathering and documenting data pertinent to resident assessment and evaluation.
- Consult with Registered Nurses regarding any changes in resident conditions.
- Receive, transcribe, and implement physicians' orders.
- Ensure the execution of care plans as instructed.
- Administer medications and treatments as prescribed.
- Participate in assigning responsibilities and ensure appropriate clinical care delivery by CNA staff.
- Ensure thorough documentation and reporting of any resident accidents or incidents in accordance with Festus Manor's Policies and Procedures and state/federal guidelines.
- Exhibit leadership qualities as a Team Leader/Role Model.
- Identify and secure necessary equipment and supplies.
- Assist in evaluating subordinate staff and occasionally provide discipline/counseling as needed.
- Maintain a safe environment for the resident population.
- Promptly notify each resident's attending physician and family or responsible party of any significant health changes.
- Maintain quality assurance logs for essential equipment used in nursing care.
- Familiarize oneself with Festus Manor's Policies and Procedures for Abuse Prevention and understand the responsibility to enforce them.
- Perform additional duties as assigned by the supervisor.
- Participate in in-service and educational programs.
- Engage in continuing education required for maintaining professional certification or licensure.
- Ability to communicate effectively in English through various mediums with staff, patient families, and external customers.
- The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of this role. Reasonable accommodations may be made for individuals with disabilities.
- While performing the duties of this position, the employee is frequently required to stand and walk, occasionally required to sit, and must be able to lift and/or move up to 50 pounds.
- Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
- Occasional exposure to blood or other body fluids, fumes, airborne particles, and toxic or caustic chemicals may occur. The noise level in the work environment is typically moderate.
