Job Summary

We are seeking a Clinical Manager AIM to oversee the day-to-day operations of our Advanced Illness Management (AIM) and Palliative Care department. The successful candidate will be responsible for coordinating clinical activities, managing staff, and ensuring the delivery of high-quality patient care.

Key Responsibilities
  • Coordinate and manage all day-to-day business affairs and clinical activities related to AIM and Palliative Care
  • Ensure clinical care delivery goals are met and exceeded
  • Manage a team of AIM Clinical Coordinators, AIM NPs, and AIM RNs
  • Oversee scheduling, frequency adherence, clinical coordination, medical record maintenance, and marketing functions
  • Manage incoming calls from partners and patients
  • Ensure Visit Freq compliance, scheduling PRN visits, post-hospitalization follow-up, and communication with partners
  • Order labs, DME, and imaging as needed
  • Coordinate results with NPs and enter telephone encounters in partner EMR
  • Ensure transitions to HSP referrals to CTC for care type
Requirements
  • Registered Nurse with Home Health, Hospice, and/or Palliative Care experience preferred
  • Bachelor's degree in nursing, Business Administration, or related field; or equivalent experience and knowledge in Health Care Operations required
  • Master's degree in business or related field preferred
  • Minimum of 2 years health care administration/operations management required
  • Must be able to work some nights/weekends on call
  • Certified in Hospice and Palliative Care - preferred
