Join CenterWell Home Health, a leading provider of home health care services, as a Licensed Practical Nurse (LPN) in a full-time capacity. As a key member of our care team, you will play a vital role in delivering high-quality patient care and support in the comfort of patients' homes.
About the Role:- Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
- Deliver skilled nursing interventions to patients with chronic conditions or recovering from illness, injury, or surgery, with a focus on preventative care and rehabilitation.
- Collaborate with the RN Supervisor to develop and implement individualized patient care plans, ensuring timely and effective communication with patients, physicians, and other healthcare team members.
- Maintain accurate and detailed documentation of patient care, including visit/shift notes, to ensure seamless care coordination and continuity.
- Participate in care coordination activities and discharge planning to ensure a smooth transition of care for patients.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
