Licensed Vocational Nurse

At Adventist Health, we are committed to delivering high-quality patient care in the comfort of our patients' own homes. As a Licensed Vocational Nurse (LVN) on our Home Health team, you will play a vital role in providing specialized nursing care to patients in need of medical attention.
Job Summary:As an LVN, you will work under the supervision of a Registered Nurse (RN) to provide general nursing care and related services to assigned patients. Your responsibilities will include performing procedures that require specialized training, participating in the planning and provision of patient care, and using knowledge of asepsis in the performance of treatments.
Key Responsibilities:- Provides care for assigned patients, under the supervision of an RN, according to established standards and nursing process.
- Performs routine visits based on departmental specific policy and procedures.
- Assists in the development of a prioritized interdisciplinary plan of care on each patient.
- Implements and coordinates plan of care utilizing appropriate organizational skills and reporting mechanisms.
- Implements individual teaching plans, including Patient Teaching Protocols and discharge Plans, based on mutual goal setting and with emphasis on patient decision making.
- Performs assigned clinical procedures (i.e. phlebotomy, injections, drug screens, etc.).
- Administers medications.
- Orders/restocks supplies as assigned, according to established par levels.
- Maintains neatness/cleanliness in assigned areas.
- Licensed Vocational Nurse (LVN) licensure in the state of practice: Required
- Basic Life Support (BLS OR HS-BLS OR RQIBLS): Required
- Health Care Provider certification: Required
- Arrhythmia certification: Preferred
- Valid Driver's License (DL) and must be at least 21 years of age or older: Preferred
