Rehabilitation Liaison Specialist

This position serves as a liaison between internal and external referral sources, and acute inpatient rehabilitation operations. The Clinical Rehabilitation Specialist is responsible for establishing relationships with case managers, physicians, payors, and other referral sources. The Clinical Rehabilitation Specialist will ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Management and the Rehab Medical Director.
Key Responsibilities- Coordinate referrals and admissions for inpatient rehabilitation services
- Establish and maintain relationships with referral sources, including case managers, physicians, and payors
- Conduct clinical screenings and assessments to ensure appropriate patient placement
- Collaborate with hospital Case Management and Rehab Medical Director to ensure smooth referral and admission process
- Provide education and support to referral sources on inpatient rehabilitation services and benefits
- Monitor and analyze data related to referral opportunities and patient outcomes
- Licensed Professional in a clinical healthcare specialty (e.g. RN, PT, OT, or ST)
- BCLS certification (AHA)
- Current state licensure in clinical practice area or eligible for state license transfer
- Graduate of an accredited school in clinical specialty
- Professional appearance and effective communication skills
- Computer and keyboarding skills (25 wpm minimum)
- Ability to work with others to ensure quality patient care
- Organization and time management skills
- Strong problem-solving skills
- Interpersonal skills
- Minimum 2 years recent clinical care experience
- Pass CARE tool credentialing, when available via UDS
- If RN, CRRN preferred and/or achieve within 2 years of accepting position
- Clinical experience in the care and management of inpatient rehabilitation patients
- Experience as a clinical liaison coordinating admissions for inpatient rehabilitation services
