Primary Care Physician Assistant/Nurse Practitioner

Job SummaryWe are seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to join our team in the Atlanta, Georgia area. As a Primary Care Clinician, you will provide high-quality care to homebound and palliative care patients, applying medical techniques and principles under the supervision of a physician. You will assess patient health, develop treatment plans, and communicate effectively with office staff and clinical operations.
Key Responsibilities- Provide primary care to patients in the Atlanta area, focusing on chronic diseases and limited mobility.
- Assess patient health through physical examinations, medical histories, and creating personalized care plans.
- Effectively communicate with office staff and clinical operations across various internal platforms.
- Develop impressions, diagnoses, and treatment plans for patients.
- Foster trust and consult with patients, families, and caregivers during visits.
- Order diagnostic testing as medically necessary and prescribe medications, supplies, and interventions.
- Board Certified, with a valid NP or PA certification in the State of Georgia.
- Strong clinical skills, with the ability to accurately and confidently treat, assess, and diagnose.
- Positive demeanor and strong people skills.
- Ability to confidently problem solve and operate independently.
- 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required).
- A flexible schedule, allowing you to see patients during a schedule that works for you.
- Autonomy, enabling you to provide the best care possible.
- Support from our in-house clinical team with any requests or orders.

- $105,000- $145,000+ (depending on experience and other incentives) (full Time).
