Position Overview

The Nurse Navigator plays a crucial role in collaboration with Physicians and Leadership to act as a bridge for the community, including referring physicians, patients, and their families, as well as other healthcare providers. This position is responsible for assessing, planning, implementing, and evaluating patient care in accordance with established organizational protocols. Early engagement with patients is essential to remove obstacles to prompt diagnosis and treatment through effective coordination of referrals from healthcare professionals and facilities. A significant aspect of this role involves providing comprehensive patient education regarding their conditions and treatment plans, as well as connecting them with community resources and support systems.

Key Responsibilities

Ensure the delivery of safe and effective patient care by closely collaborating with physicians across various specialties to coordinate and communicate care plans.
Foster a collaborative and efficient transitional process for patients and families, working alongside community physicians and related services.
Collaborate with multidisciplinary teams to maintain and implement coordinated patient care, ensuring effective communication among all healthcare team members.
Develop and adhere to triage protocols for referring patients to appropriate specialists for diagnosis and treatment, assisting families in securing necessary referrals and appointments.
Work with MD directors to establish parameters for assessing patient risk based on complexity.
Review patient records to facilitate appropriate scheduling prior to initial visits.
Proactively enhance discharge protocols to standardize processes, ensuring smooth transitions for inpatient and outpatient experiences, including support for returning to school.
Communicate effectively with referring physician offices regarding patient and physician needs before and after services are rendered, serving as a liaison to community organizations for information and support.
Address patient inquiries and concerns related to their care plans through timely communication.
Educate patients about their conditions and provide informative materials to families, developing care plans that enhance understanding and satisfaction while addressing clinical concerns.
Collaborate with marketing and outreach teams to inform referring physicians and facilities about available services.
Participate in the screening and referral process for patients eligible for clinical research trials.
Act as a nurse liaison to facilitate coordination of care within the brain wellness clinic, ensuring a seamless experience for patients throughout their healthcare journey.
Ensure effective communication between staff and physicians, managing appointment arrangements and follow-ups, while serving as a primary resource for obtaining medical records and test results.
Serve as a clinical expert and role model within the department, identifying and addressing clinical and communication challenges.
Utilize collected data to integrate evidence-based practices into patient care processes.
Maintain comprehensive records of physician and patient inquiries, tracking outcomes related to communication timeliness, completed referrals, and overall results.
Disseminate new information throughout the institution and community, staying current with advancements in the specialty.
Provide patients with tailored resources, including support groups and therapy options.
Promote a safe working environment by consistently performing thorough hand-offs and supporting infection prevention and patient safety measures.

Qualifications

Bachelor's Degree in Nursing
Active RN Licensure in the State of Florida or Multi-State Enhanced Nursing License Compact (eNLC)
Current American Heart Association BLS certification
2-4 years of experience in Pediatric Nursing

Skills and Competencies

Master's degree in Nursing is preferred.
Specialty certification is preferred.
Experience in pediatric nursing within an acute care environment is preferred.
Strong interpersonal and communication skills.
Ability to communicate effectively in both verbal and written formats.
Proficient in conflict resolution skills.
Strong analytical abilities.
