Job Description: As a Patient Advocacy Specialist in the Emergency Department, you will serve as a liaison between patients, families, and healthcare providers to ensure that patients receive high-quality, patient-centered care. You will utilize your clinical expertise, communication skills, and knowledge of case management methodologies to promote positive patient outcomes.

Key Responsibilities:

  • Develop and implement individualized care plans that address patient needs and goals
  • Coordinate care services, including arranging for transportation, interpreting services, and other support resources as needed
  • Communicate effectively with patients, families, and healthcare providers to ensure comprehensive understanding of treatment plans and progress
  • Maintain accurate and timely records of patient interactions, progress, and outcomes

Education and Experience: A Bachelor's Degree in Nursing is required; or an Associate's Degree in Nursing with six years of Case Management experience may be considered in lieu of a Bachelor's Degree. A minimum three years of bedside nursing experience in a hospital setting and one year of Case Management experience are preferred.

Knowledge and Skills:

  • In-depth knowledge of case management practice, methodologies, and principles
  • Substantial knowledge of basic nursing concepts, patient assessment, and care coordination
  • Strong analytical, problem-solving, and decision-making skills
  • Excellent communication, interpersonal, and leadership abilities
  • Able to work independently with minimal supervision and prioritize multiple tasks effectively

Licensures and Certifications: An active California Registered Nurse (RN) license is required; Basic Life Support (BLS) certification from the American Heart Association is also necessary.
