About the Role:

Key Responsibilities:

* Conduct thorough assessments of patients' health status and develop individualized care plans that address their physical, emotional, and social needs.* Work collaboratively with healthcare teams to coordinate care and ensure seamless transitions between departments and services.* Administer medications and treatments according to established protocols and document patient responses and outcomes accurately.* Participate in quality initiatives to improve patient outcomes and reduce healthcare disparities.* Foster positive relationships with patients, families, and colleagues to promote a culture of respect, empathy, and compassion within our organization.

Requirements:

* A current license as a Vocational Nurse in the State of Texas is mandatory.* Current CPR certification is necessary.* Strong analytical and problem-solving skills, along with excellent communication and interpersonal skills, are crucial for success in this role.* Prior experience working in a community healthcare setting is highly valued, but not a requirement.
