LPN - Pace Caregiver

As a Licensed Practical Nurse (LPN) on our Pace team, you will play a vital role in providing high-quality care to our participants. Under the direction of the Clinic Services Supervisor, you will use your assessment skills to identify the needs of our participants and develop individualized care plans. You will administer medications, implement primary care orders, and evaluate participant complaints. Your excellent communication skills will enable you to educate participants and caregivers about care plans, nursing treatments, and medications. You will also communicate participant conditions to primary caregivers, physicians, and the multidisciplinary team.
Requirements
Graduate of an accredited school of nursing
Current LPN license in good standing to practice in the required state
Hmong speaking preferred
IV Certification preferred
Experience with paperless documentation preferred
1 year of experience working with a frail or elderly population preferred
Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance
Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact
Skills and Abilities
Broad knowledge base of health care problems of the frail elderly
Knowledge of medical equipment and instruments
Knowledge of common safety hazards and precautions to establish a safe work environment
Skilled in identifying problems and recommending solutions
Skilled in preparing and maintaining records and writing reports, and responding to correspondence
Skilled in developing and maintaining department quality assurance
Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members, and family caregivers
Ability to react calmly and effectively in emergency situations
Knowledge of medications used in geriatrics and adverse reactions
Skilled in wound care
