We are seeking a dedicated Hospice Licensed Vocational Nurse (LVN) to join our interdisciplinary team at Interim HealthCare of Sacramento. As a Hospice LVN, you will provide compassionate end-of-life care to patients and their families in the comfort of their own homes.

About the Role

As a Hospice LVN, you will work as part of a collaborative team to deliver high-quality patient care. Your responsibilities will include:

  • Providing home-based care to patients with a prognosis of six months or less to live, focused on quality of life
  • Working with an interdisciplinary team to carry out the patient's plan of care
  • Performing nursing services planned, delegated, and supervised by the registered nurse
  • Assessing and providing patient and family/caregiver education and information pertinent to diagnosis and plan of care
  • Taking a holistic approach to care that engages the patient's mind, body, spirit, and family
Requirements

To be successful in this role, you will need:

  • Graduation from an accredited vocational nursing program and active LVN license in California
  • One year of hospice LVN experience
  • CPR certification
  • Valid California driver's license, auto insurance, and reliable transportation
  • Knowledge of state and federal home health and hospice regulations
  • Clinical proficiency in nursing, strong communication skills, and compassion
