Job Title: Orthopedic Nurse Navigator

At Yale New Haven Health, we are seeking a highly skilled and compassionate Orthopedic Nurse Navigator to join our team. As a key member of our multidisciplinary team, you will play a vital role in ensuring exceptional patient care, patient experience, and clinical outcomes for our Total Joint Replacement patients.

Responsibilities:
  • Collaborate with Service line leadership to review comprehensive risk assessments and coordinate risk modification programs for high-risk patients.
  • Direct the process of assessment, planning, facilitation, and advocacy for options and services to meet patient health needs.
  • Optimize patients with risk modification programs as designated by the Service line Leadership team.
  • Coordinate and assign referrals according to defined criteria, including location and special requirements.
  • Promote the use of tele-rehabilitation and collaborate with inpatient and outpatient therapists for follow-up.
  • Provide accurate and comprehensive assessment of relevant medical history and condition, including physical assessment and review of pertinent laboratory results, mental status, and psychosocial support.
  • Support collection and documentation in the EMR of Patient Reported Outcomes.
  • Review medication reconciliation and communicate with primary care provider and surgeon regarding optimization of patient.
Qualifications:
  • Bachelor's Degree in Nursing required; Master's degree in Nursing preferred.
  • Three to five years nursing experience in Orthopaedic nursing, with experience in clinical care, clinical process improvement, outcomes management, case management, and physician relations.
  • Excellent oral and written communication skills.
  • Connecticut RN license required.

  • Ability to thrive in a fast-paced, complex academic medical environment.
  • Background in Total Joint Arthroplasty required.
  • Understanding of clinical processes.
  • Self-motivated, goal-oriented, and able to work independently.
  • Excellent assessment, communication, and organizational skills.
  • Ability to manage and follow through on multiple priorities.
  • Proficient in EPIC and Excel.
