Clinical Pharmacist-Pediatrics Specialist

This role assumes responsibility for the medication management of assigned patients, ensuring appropriateness based on the patient's health status, institutional criteria, and cost-efficient measures. The successful candidate will remain integrally involved in the education of Pharmacy and Medical Residents, providing education to healthcare providers as needed, and contributing to the development of pharmacy-related documents.
Key Responsibilities- Provides pharmacokinetic consultation, utilizing laboratory data to evaluate drug selection and dosing, and communicates findings to healthcare practitioners.
- Evaluates patient response to drug therapy, documenting interventions and conducting outcome studies/evaluations.
- Assists in the development of pathways, best practices, and guidelines for assigned populations.
- Educates staff, students, patients, healthcare providers, and other healthcare professionals, submitting publications as required.
- Mentors residents, students, and new staff to meet departmental goals.
- Ensures overall patient care beyond hospital visits, through all transitions of care.
- Accommodates special needs of the organization, volunteering for tasks during staffing shortages.
- Adapts behavior to specific patient populations, respecting privacy and communicating effectively.
- Current Pharmacy license in the state of practice.
- Completion of a PGY1 Pharmacy Practice Residency (3 years related specialty experience accepted in lieu of PGY1 Residency).
- Preferred: Doctor of Pharmacy (PhD) and Completion of a PGY2 Specialty Pharmacy Residency.
The physical demands of this role include exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. The incumbent works with patients who have known or suspected communicable diseases and may enter isolation rooms, with potential exposure to hazardous medications and waste.
