Empowering Patients through Personalized Care

A Home Health Aide plays a vital role in providing direct patient care under the guidance of a Registered Nurse and according to the Aide Plan of Care (POC). Key responsibilities include:

  • Assisting patients with self-administered medications, ensuring accurate dosages and observing medication intake.
  • Accurately taking temperature, pulse, and blood pressure measurements, recognizing and reporting abnormal results to supervisors.
  • Helping patients maintain good personal hygiene through bathing, grooming, skin care, and other essential activities.
  • Assisting with feeding patients, communicating basic nutrition principles, and observing food and fluid intake when necessary.
  • Supporting patient mobility, exercises, positioning, transfers, and ambulation as per the Plan of Care and CenterWell Home Health policy.
  • Reporting changes and documenting pertinent information and care rendered to patients, ensuring continuity of care.
  • Practicing acceptable infection control principles and providing a clean, safe, and comfortable environment.
  • Assisting with household duties, including light laundry, bed changing, and meal preparation, when necessary.

CenterWell Home Health specializes in personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, or hospitalization. Our care teams include nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers, all working together to help patients rehabilitate, recover, and regain their independence.

Requirements:

  • High school diploma or equivalent
  • Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
  • Must meet applicable state certification requirements
  • A valid driver's license, auto insurance, and reliable transportation are required
  • Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
  • At least one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
