Compassionate Licensed Practical Nurse

Cedarhurst Senior Living, a prominent provider in the healthcare sector dedicated to serving older adults, is seeking a committed and career-focused individual to fulfill the role of Licensed Nurse (LPN) within our community.
As a Licensed Nurse, you will play a vital role in enhancing the daily lives of our residents. Your responsibilities will encompass the planning, execution, and assessment of all nursing care prescribed and delivered to our residents.
Your Impact as a Licensed Nurse:
- Delivering care in accordance with the nurse practice act and adhering to state and federal regulations for nursing facilities.
- Collaborating with the Director of Nursing to guide and supervise care staff in addressing residents' needs.
- Conducting and updating assessments and care plans during admission, periodically as required, and when there are changes in residents' physical or psychological conditions.
- Transcribing and reviewing physician orders, administering medical treatments as directed, and assisting physicians during resident examinations as necessary.
- Creating individualized education plans for residents, as directed by physicians and community policies, including instruction for residents and their families.
- Monitoring residents and communicating any changes in their condition to physicians, healthcare proxies, and supervisors, providing timely interventions as needed.
- Administering and documenting medications accurately, ensuring compliance with the five rights: right medication, right route, right resident, right time, and right dose.
- Overseeing and monitoring self-administration of medications by residents when applicable.
- Documenting all relevant observations, incidents, interventions, and clinical procedures throughout the shift as required.
- Assisting residents in meeting their nutritional and hydration needs, accurately documenting on the nursing record.
- Supporting residents with daily living activities, including personal hygiene, oral care, dressing, and toileting.
- Coordinating medical, dental, and laboratory appointments for residents.
- Implementing proactive safety measures and monitoring during rounds.
- Responding promptly to residents' call lights.
- Assisting residents with mobility and transfers using proper techniques.
- Taking and recording vital signs to identify any deviations from normal conditions.
- Maintaining a restraint-free environment at all times.
- Supporting fall prevention initiatives for residents requiring assistance.
- Reacting appropriately to emergency situations based on established nursing standards and protocols.
- Engaging in duties that may include providing first aid and contacting emergency services when necessary.
- Reviewing service plans for assisted living and memory care residents.
- Encouraging resident participation in scheduled activities and assisting with transportation as needed.
- Providing post-mortem care as instructed.
- Participating in the management of the medication delivery system, ensuring timely order entry, tracking, and documentation.
- Promoting the physical, personal, and emotional well-being of each resident, striving to help them achieve maximum independence.
- Understanding and respecting residents' advance directives and preferences.
- Facilitating effective communication between community staff and residents' families regarding care and well-being.
- Collaborating effectively with colleagues and management, demonstrating initiative and conflict resolution skills as needed.
- Meeting the physical demands of the role, which may include standing, moving, and lifting up to 50 pounds independently, and up to 200 pounds with assistance.
- Performing additional duties as assigned.
Qualifications for the Licensed Nurse Role:
- Graduation from an accredited nursing program and possession of a current state license for LPN or RN.
- Preferred experience of 1+ years in nursing, though new graduates are encouraged to apply.
- CPR and BLS certification preferred.
- Proficiency in using internet browsers, Microsoft Office applications, electronic health records (EHR), and any relevant HR systems.
