Key Responsibilities:

  • Collaborate with the Interdisciplinary Team to assess patient needs and develop individualized care plans.
  • Attend team meetings to gather information, communicate changes, and maintain accurate records.
  • Coordinate care plan conferences with the interdisciplinary team, patients, and families.
  • Evaluate patient status, nursing instructions, and outcomes of care provided.
  • Develop patient education plans according to individual needs, as prescribed by physicians and hospital procedures.
  • Participate in resident/patient and family education.
  • Ensure accurate and timely data entry into computerized MDS programs.
  • Maintain knowledge base related to data entry and computer technology.
  • Ensure all supporting documentation is complete and accurate.
  • Document patient status according to unit-specific procedures.
  • Demonstrate clinical expertise in assessing patient needs, potential problems, and implementation of solutions in accordance with state and federal guidelines.
  • Participate in unit performance improvement activities.
  • Provide backup support for Admission Coordinator in their absence.

Requirements:

  • Requires an Associate's (A.A.) or Bachelor's degree (BSN) from an accredited school of Nursing.
  • BCLS, NC RN License
  • Two years long term care experience plus one year of MDS experience.

Knowledge and Skills:

Language Skills:
Ability to read, analyze, and interpret clinical information and technical procedures. Ability to document clinical information. Ability to effectively communicate information and respond to questions from patients, physicians, family members, and other UNC staff.

Mathematical Skills:
Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations

Reasoning Ability:
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions.
