LPN (Home Care/Clinic Rotation) - PACE Elder Care Services Professional

Cambridge Health Alliance is seeking a skilled Licensed Practical Nurse (LPN) to join our PACE Elder Care Services team. As a Home Care LPN, you will provide coordinated nursing care to program participants in their own homes, working collaboratively with the RN and Interdisciplinary Team to assess, develop, and implement comprehensive care plans.
Key Responsibilities:
• Provide nursing care to program participants in their homes, utilizing evidence-based interventions to support comprehensive care plans.
• Communicate participant status to the RN, including new problems and/or significant changes in status, to inform best practices for continued care.
• Collaborate with the RN and IDT to assess, develop, and implement participant care plans.
Requirements:
• Graduation from an accredited licensed practical school of nursing.
• 1-year experience in geriatrics preferred.
• 1-2 years home care experience preferred.
• Maintains current Basic Life Support (BLS) certification.
