Job Summary

As an Inpatient Case Manager RN, you will be responsible for providing comprehensive care management services to patients across the healthcare continuum. Your primary focus will be on patient transition to the appropriate level of care while ensuring clinical appropriateness and length of stay.

Key Responsibilities:
  • Directly support patients identified in need of comprehensive case management in the hospital setting.
  • Promote effective utilization and monitoring of health services, collaborating and communicating with the healthcare team and patient/caregiver to manage care and transitions.
  • Develop and/or implement a comprehensive care plan based on assessment and evaluation to include physical, mental, and psychosocial needs of the inpatient/caregiver.
Requirements:
  • 18 months of nursing experience required.
  • BLS certification within 90 days of hire.
  • BSN preferred; all Registered Nurses without a BSN will be required to sign a BSN agreement committing to successfully obtain their BSN within 5 years of hire.
  • Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
Qualifications:
  • N-2YR - RN-Associate's Degree (Required)
  • N-3YR - RN-Diploma (Non-degree) (Required)
  • N-4YR - RN-Bachelor's Level Degree (Required)
  • N-6YR - RN-Master's Level Degree (Required)
