BAYADA Home Health Care is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide exceptional clinical care and relationship management to our adult clients in Northern Virginia.

The ideal candidate will have a current license as a Licensed Practical Nurse in the state of Virginia, graduation from an accredited and approved nursing program, and prior home care experience. Strong interpersonal skills, solid computer skills, and the ability to work independently and manage time effectively are essential.

Responsibilities:

  • Make home visits to clients living in designated territories.
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
  • Monitor clients' conditions; report changes to Clinical Manager and Client Services Manager.
  • Follow up with, execute, and properly document doctors' orders.
  • Perform client assessments as necessary.
  • Case management and coordination.
