Job Description

We are seeking a highly skilled Care Transition Navigator to join our team at Methodist Richardson Medical Center. This role will play a critical part in ensuring quality outcomes, patient throughput, and discharge planning.

Responsibilities
  • Coordinate activities that promote optimal care and resource utilization
  • Identify potential barriers to patient throughput and minimize delays in discharge plans
  • Communicate clearly and openly with colleagues and patients
  • Build relationships to promote a collaborative environment

Educational Requirements: Bachelor's degree in Social Work or Master's degree in Social Work with Texas Social Work license required. Registered Nurse with BSN preferred.

Experience: 1 year of experience in a health-related setting, with hospital case management experience preferred.

About Us: Methodist Richardson Medical Center is a 443-bed acute care hospital serving Richardson, Garland, Plano, and surrounding areas. We offer a range of services, including behavioral health and addiction recovery, inpatient rehabilitation, and long-term acute care.

Estimated Salary: $65,000 - $85,000 per year, based on location and experience.
