The Thresholds Homeless Outreach Program is seeking a highly skilled and compassionate Street Outreach & Engagement Specialist to join our team. As a key member of our outreach team, you will work collaboratively to engage and support individuals experiencing homelessness with serious mental illnesses.
Key Responsibilities- Provide Street Outreach and Engagement Services: Conduct outreach and engagement activities on CTA platforms and surrounding areas to assess and provide linkage services to housing, benefits, treatment, advocacy, and other identified needs.
- Develop and Implement Service Plans: Collaborate with members and the outreach team to develop and implement member-driven service plans to meet identified goals.
- Provide Case Management and Support: Provide case management, job support, crisis intervention, and psycho-social rehabilitation services in the community.
- Assess and Investigate Resources: Assess the need for service, investigate current and available resources, and make recommendations for treatment.
- Support Member Self-Advocacy: Support and promote member self-advocacy and empowerment.
- Document and Maintain Records: Maintain timely and accurate documentation of services provided.
- Education and Experience: Bachelor's degree preferred; previous outreach experience and lived experience a plus.
- Skills and Abilities: Utilize a member-centered approach, strong engagement techniques, empathy, and patience; ability to work with diverse populations in various settings; demonstrated interpersonal skills and ability to work collaboratively on a team.
- Physical Demands: Willingness to work Monday-Friday, 4:00pm-12:30am; valid driver's license required.
- Competitive Pay: $24.25 hourly / $49,400 - $50,440 annually.
- Benefits: Generous Paid Time Off Policy, 4 Medical Insurance Plans, Dental Insurance, Vision Insurance, 403(b) with 3% employer match, Mileage Reimbursement, Public Service Loan Forgiveness, Supervision for Clinical Licensure at no cost.
