Job Summary

We are seeking a skilled Nurse Practitioner to join our team of compassionate healthcare professionals. As a key member of our geriatric care team, you will provide primary, urgent, and acute medical care to patients in their homes. Your expertise in end-of-life, geriatrics, internal medicine, and palliative care will be invaluable in delivering concierge-level care to our patients.

Responsibilities
  • Provide primary care to patients in their homes, including routine check-ups, medication management, and health education.
  • Collaborate with our interdisciplinary team to develop and implement comprehensive care plans for patients.
  • Conduct initial patient screenings, physical examinations, and create assessment plans.
  • Address urgent care needs of patients, including emergency situations and hospitalizations.
  • Order, interpret, and analyze diagnostic tests, and determine the need for follow-up appointments and further treatment options.
  • Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents.
  • Update patient records and ensure accuracy at each patient appointment.
  • Deliver quality care while maintaining company metrics on documentation, coding, HEDIS measures, patient engagement, and medical expense.
  • Participate in regularly scheduled team meetings and maintain required licensure and certification.
Requirements
  • Master's Degree in Nursing
  • Valid nurse practitioner license in state of operations and current board certification in your field
  • 5+ years of progressive care experience
  • BLS and ACLS certified and an NPI# as an NP
  • Valid driver's license, car insurance, and willingness to drive to patient homes/patient location
