Advanced Practice Provider

At Oak Medical, we are seeking an experienced Advanced Practice Provider (APP) to deliver high-quality medical care in skilled nursing facilities. This role is focused on providing acute and chronic care management for adult and geriatric patients.
Key Responsibilities:
- Deliver primary care for adult and geriatric patients, addressing acute and chronic health conditions.
- Develop personalized treatment plans in close coordination with the facility care team and other healthcare providers.
- Establish medical diagnosis, prescribe medications, perform diagnostic and therapeutic procedures, and other care coordination as needed.
- Ensure timely, accurate, and thorough documentation of patient assessment and care plans.
- Work closely with specialty providers to coordinate integrated care when needed.
- Communicate changes in patient conditions and treatment plans to the clinical team.
- Communicate with patients, families, and staff to support understanding of care plans and interventions.
- Foster strong relationships with patients, their families, and the facility staff to enhance care quality.
Qualifications:
- Advanced Practice Provider license (NP or PA).
- Valid Wisconsin APNP or PA-C License.
- Valid DEA.
- AHA BLS certified.
- Minimum 1 year Advance Practice Provider experience (preferred).
- Experience working in skilled nursing facilities or similar settings is preferred.
- Strong diagnostic, clinical, and interpersonal skills.
- Ability to work independently and collaborate with interdisciplinary teams.
