Job Title: LPN Home Health Nurse

Covenant Health Inc. is seeking a skilled LPN Home Health Nurse to join our team. As a LPN Home Health Nurse, you will provide high-quality nursing care to patients in their homes, working closely with our interdisciplinary team to ensure comprehensive care.

Responsibilities:

  • Provide nursing care to assigned patients in their homes, under the supervision of a Registered Nurse.
  • Administer care with other disciplines, including physical therapy, occupational therapy, and social work.
  • Collaborate with patients, families, and caregivers to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
  • Document patient progress, including medication administration, vital signs, and other relevant information.

Requirements:

  • Tennessee LPN license or Licensed Vocational Nurse.
  • One year of professional experience in hospital or prior home health experience.
  • Successful completion of LPN advanced skills course.
  • CPR certification.
  • Valid Tennessee driver's license and state-mandated minimum insurance coverage.
