This position requires a graduate degree from an accredited physical therapy curriculum with certification in physical therapy or foreign-trained equivalent. Job DescriptionIn this role, you will be responsible for the physical therapy management of patients under the direction and supervision of the Rehabilitation Services Director. Your duties will include initiating discharge planning from physical therapy, implementing treatment programs through direct patient treatment, and coordinating care with other health care professionals. You will also be responsible for providing accurate, timely documentation of patient management and ensuring that patients receive maximum benefit from physical therapy.
Key Responsibilities:- Develop and implement individualized treatment plans based on patient assessment and goals
- Conduct regular progress evaluations to assess patient response to treatment and adjust treatment plans as needed
- Communicate effectively with patients, families, and interdisciplinary teams to ensure seamless care coordination
- Provide education and training to patients and families on physical therapy techniques and self-care strategies
To be successful in this role, you will need:
- A graduate degree from an accredited physical therapy curriculum with certification in physical therapy or foreign-trained equivalent
- Strong communication and interpersonal skills, with ability to work effectively with diverse populations
- Ability to work in a fast-paced, dynamic environment and adapt to changing priorities
- Excellent time management and organizational skills, with ability to prioritize tasks and meet deadlines
