LHC Group is seeking a highly skilled and compassionate Patient Care Manager to join our team. As a Patient Care Manager, you will play a vital role in ensuring the delivery of high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities:

  • Supervise and coordinate clinical services to ensure seamless patient care.
  • Provide and direct provisions of nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies.
  • Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
  • Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
  • Review assessments and plans of care daily, per assigned workflow, and consult clinicians with recommendations, as appropriate.

Requirements:

  • Current RN licensure in the state of practice.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
