About the Role

We are seeking a compassionate and skilled Hospice Registered Nurse RN Case Manager to join our team at AccentCare, Inc.

Key Responsibilities
  • Provide high-quality patient care and support to individuals and their families in a hospice setting
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
Requirements
  • Graduated from an approved school of professional nursing
  • One year experience as a registered nurse, preferred
  • Licensed to practice as a registered nurse in the state of agency operation
  • Possess and maintain valid CPR certification while employed in a clinical role, preferred
  • Must be a licensed driver who can travel to all business locations
