Job Opportunity

We are seeking a dedicated and compassionate Registered Nurse to join our team as a Hospice Case Manager. This role will involve providing high-quality patient care and support to individuals and their families in Alameda and Contra Costa Counties.

Key Responsibilities:

  • Deliver patient-centered care to hospice patients and their families
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Provide education and support to patients and their families

Requirements:

  • Graduated from an approved school of professional nursing
  • One year of experience as a registered nurse, preferred
  • Licensed to practice as a registered nurse in the state of agency operation
  • Possess and maintain valid CPR certification while employed in a clinical role, preferred
