A skilled practitioner responsible for overseeing, assessing, and delivering physical therapy services to patients in their residences, adhering to medical directives.
1. Conduct comprehensive evaluations of patients utilizing diagnostic assessments related to muscle, nerve, joint, and functional capabilities.
2. Formulate an initial assessment of the patient's health status and perform regular re-evaluations, creating a detailed problem list and care plan that outlines treatment types and frequencies.
3. Suggest adjustments to the care plan to the physician as necessary.
4. Maintain accurate clinical documentation on the day of service and ensure all records are current.
5. Provide treatment aimed at addressing impairments and enhancing functional independence.
6. Educate patients and their caregivers on the proper use of assistive devices such as prosthetics, orthotics, and durable medical equipment (e.g., canes, walkers, wheelchairs).
7. Involve and inform patients and caregivers about the treatment strategy and objectives.
8. Attend departmental meetings and participate in training sessions.
9. Engage in case discussions and contribute to the ongoing development and periodic updates of the physician's treatment plan.
10. Ensure adherence to safety protocols and proper maintenance of therapeutic equipment.
11. Perform additional duties as assigned.

Graduation from an accredited Physical Therapy program is required.
Qualifications
Possession of a current, valid Physical Therapy license issued by the relevant state department of education.
A minimum of one year of hands-on experience as a licensed physical therapist is preferred.
Work Environment
The role primarily occurs in non-office settings, with potential exposure to outdoor conditions.
Physical Demands
This position is classified as medium work, requiring the ability to exert up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects, including patients.
Medium work involves both sitting and standing.
