Job Title: Enhanced Service Coordinator

St. Elizabeth's Medical Center is seeking a highly skilled Enhanced Service Coordinator to join our team. As a key member of our staff, you will provide enhanced service coordination to individuals with developmental disabilities and their families.

Job Summary:

The Enhanced Service Coordinator will work under the direct supervision of the Program Manager to provide enhanced service coordination to individuals with developmental disabilities and their families. This role requires a strong understanding of family systems and human dynamics, as well as excellent conflict resolution skills.

Key Responsibilities:
  • Maintain an average service coordinator to-consumer ratio of 1-to-40 for consumers with lowest per capita and/or no POS expenditures in order to enhance relationships between consumer/family.
  • Empower individuals to partner with IRC and other community agencies and to assist in identification of new resources that are culturally relevant to our community.
  • Work to identify barriers in accessing services and supports, including:
    • Use PCP strategies for the development and implementation of the consumer's IPP, progress reports, the consumer's annual reviews.
    • Provide follow-up, reassessments, and periodic review of consumer's progress and program effectiveness.
    • Assist in locating and developing natural supports.
    • Initiate the purchase of vendored and generic services and monitor service delivery as identified in the IPP.
    • Advocate on social, legal, education, mental health and/or forensics issues related to the consumer. Participate in outreach and represent the agency in community meetings and events.
  • Provide services to consumers and perform liaison activities as required. Assure consumers' rights and dignity are maintained in the provision of services.
  • Maintain contact with consumers and their families at a minimum on a monthly basis or more. Provide information and referral services.
  • Facilitate services identified in Individual Program Plan (IPP) and/or Interdisciplinary Team (IDT) meetings. Coordinate, implement, and update the Annual Review and IPP process on an annual basis.
  • Evaluate consumer progress and plan as identified in IPP on an ongoing basis to include need for community placement. Review consumer assessment including medical, psychological/ developmental, educational or vocational and others as needed.
  • Seek out and effectively utilize generic resources on behalf of consumers.
  • Under the direction of the Program Manager, do individual or family crisis intervention and appropriately document activities.
  • Complete Special Incident Reports on a timely basis and inform representatives of other agencies and programs as appropriate.
  • Identify situations in vendored programs requiring the attention of Quality Assurance and/or Resource Development and Transportation Program and make appropriate referrals.
  • Complete appropriate Targeted Case Management (TCM) documentation accurately and on a timely basis.
  • Medicaid Waiver standards must be maintained at all times.
  • Represent the regional center at case conferences and inter-agency meetings.
  • Keep Program Manager informed of the status of the caseload; consumer needs and progress; liaison activities, and unusual or difficult case situations. Provide monthly written reports of activities to the Program Manager.
  • Seek to maintain current and relevant knowledge base. Attend training sessions as required.
  • Upon request, assist in case management coverage to consumers whose Consumer Services Coordinator is absent.
  • Daily, complete administrative requirements and Consumer File Record Documentation in accord with Agency's Policies and Procedures. All necessary paperwork, reports, etc. must be completed on a timely basis. Completion of at least 95% required case-related paperwork within designated time frames.
  • Be well prepared for Compliance Review, eligibility review and other administrative case reviews. In the event part of the record is out-of-date, plan to correct the deficiency immediately.
  • Use equipment accurately and keep it in good repair. Keep work area neat and orderly and free from hazards. Observe all safety rules.
  • Utilize agency's IT systems as assigned, maintaining security and following agency's protocol and procedures, rules and requirements.
  • On a daily basis, complete administrative requirements and all requests for action including, but not limited to, those related to Eligibility Review, Special Incident Reports (SIR) closures, and Quality Alerts on a timely basis.
  • Maintain good attendance and punctuality.
  • Work cooperatively and effectively with others. Perform as a member of the team, answer questions, share expertise and contribute to the harmony of the team.
  • Organize travel efficiently and effectively. Maintain a safe driving record. Comply with the Agency's Affirmative Action Plan. Comply with the Agency's Personnel Policies and Procedures. Comply with Agency's Injury and Illness Prevention Plan. Notify Program Manager of any unsafe working conditions.
  • Perform different or additional work as assigned.
Requirements:
  • Bachelor's degree; Master's degree preferred.
  • Three-years of experience in developmental disabilities or a related field.
  • Demonstrated ability that person is a self- starter with a positive attitude, willing to adapt as projects and related work evolve.
  • Ability to handle change well and be flexible and adaptable in dealing with interruptions, new priorities, and new assignments.
  • Experience working with families from diverse cultural, linguistic and socio-economic backgrounds.
  • Must have an understanding of family systems and human dynamics.
  • Must have excellent conflict resolutions skills and has the ability to seek creative solutions.
  • Ability to relay information clearly, effectively, and accurately, both verbally and in writing. Ability to follow oral and written direction. Ability to establish and maintain effective working relationships with others. Ability to work cooperatively and effectively with others. Perform as a member of a team, answering questions, sharing expertise, and generally contributing to the harmony of the team.
  • Exceptional organizational skills as evidenced by ability to work independently in determining priorities and completion of assignments.
  • Full use of an automobile, possession of a valid California driver's license and liability insurance for the minimum amount prescribed by law or ability to provide for independent transportation.
  • Must have and maintain a safe driving record.
  • Bilingual preferred.
