Primary Care Physician

We are seeking a highly skilled Primary Care Physician to join our team at CenterWell Senior Primary Care. As a key member of our healthcare team, you will be responsible for providing high-quality, patient-centered care to our senior patients.
Responsibilities- Provide comprehensive medical care to patients, including wellness visits, physical exams, and chronic condition management.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- 5+ years of experience in primary care medicine.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Strong problem-solving and critical thinking skills.
