Job Summary

Preferred Therapy Solutions is seeking a skilled Physical Therapist to join our team in a sub-acute/skilled nursing rehab department. As a Per Diem Physical Therapist, you will provide comprehensive and compassionate therapy services to our residents.

Key Responsibilities

  • Provide physical therapy services to residents in a sub-acute/skilled nursing facility setting
  • Work collaboratively with other rehab therapists and nursing staff to achieve optimal patient outcomes
  • Develop and implement individualized treatment plans to meet the unique needs of each resident
  • Conduct thorough assessments and evaluations to identify resident needs and goals
  • Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination
  • Stay up-to-date with industry developments and best practices through ongoing education and training

Requirements

  • Current Physical Therapist license or eligibility to practice in the state employed
  • Strong computer skills with Microsoft Outlook, Word, Excel, and experience with electronic documentation
  • Excellent communication and interpersonal skills
  • Ability to work well in a team environment and adapt to changing priorities
  • Flexibility with scheduling and willingness to work varied shifts
