Job Description

We are seeking a Memory Support Director to manage the 'Moments Matter' program within our community. This role is responsible for ensuring that all scheduled activities listed on the monthly calendar take place. The Memory Support Director will oversee the memory support care staff to effectively engage residents during encounters.

Essential Functions

  • Manage and maintain the Resident Well-Being binder, which includes the Get to Know Me document, Resident Well-Being Profile, and Know the 25 guidelines.
  • Ensure the implementation and ongoing management of Memory Support Signature programs, including Taste & Tell (collaboration with Chef), Community Service Project, Hydration Stations within activity space, Essential Health Program, and other programs as needed.
  • Lead activities as scheduled or oversee Memory Support Specialists and/or care staff to ensure resident engagement and daily programming that offers a mix of large and small groups, individual and parallel program opportunities.
  • Coordinate with the Director of Health and Wellness to attend family care conferences required by state regulations. Occasionally assist with assessments of potential new residents and provide Memory Support program overviews and updates.
  • Work with Resident Experience staff to ensure Memory Support residents have tailored activities and social interactions appropriate to their cognitive abilities. Utilize 'In the Moment' resources to support and train Memory Support staff, ensuring diverse activities and programs that meet resident needs.
  • Manage, coach, assist, and mentor care staff working in Memory Support to maintain positive resident outcomes, minimize behavioral conflicts, and ensure compliance with state regulations.
  • Meet monthly with the Culinary Director (Chef) to coordinate Taste & Tell, submit/discuss food function sheets for upcoming activities, and ensure Memory Support meals and snacks meet organizational standards while maintaining a calm and nurturing dining environment.

Education and Experience

  • A minimum of 4 years of management experience with employee oversight and customer relations is preferred. Experience in Dementia care settings and certifications in geriatrics, leadership, and human resources are also desirable.
  • A genuine desire to advocate for those with Dementia diagnoses is essential.
