Job Summary

VNA of Care New England is seeking a highly skilled and compassionate Home Health Clinician to join our team in Providence, RI. As a key member of our interdisciplinary care team, you will play a vital role in providing high-quality patient care and achieving positive therapeutic outcomes.

About the Role:

  • Under the supervision of the Team/Program Manager, you will be accountable for effectively managing a caseload of patients from admission to transition into the community.
  • You will coordinate interdisciplinary care, monitor treatment plans, and adjust as needed to achieve optimal patient outcomes.
  • With a focus on process improvement, you will identify opportunities to enhance patient care and contribute to the growth and development of our organization.

Requirements:

  • A Bachelor's Degree in a relevant discipline is strongly preferred.
  • A minimum of one (1) year progressive clinical experience is required, with previous experience in an acute care or home health care environment setting preferred.
  • Valid CPR certification, RI Board license, and a valid driver's license with automobile liability insurance are also required.

Estimated Salary: $65,000 - $85,000 per year
