Job Overview

As a Patient Referral Specialist at Aquacare Physical Therapy, you will play a crucial role in facilitating seamless referral and scheduling operations for our patients. Your primary duties will encompass:


Referral Management:
  • Supervise the referral management for all incoming patient referrals received via fax across multiple locations.
  • Reach out to all referrals within a day of receipt, leaving voice messages and following up with emails or texts if direct communication is not possible.
  • Accurately document essential demographic details, referral specialties, and destinations, ensuring necessary authorizations are secured.
  • Work closely with healthcare providers to prioritize urgent and routine referrals, adjusting workflows as needed.
  • Confirm that all related orders are correctly entered into the electronic medical records (EMR) system.
  • Conduct follow-up calls within a week if patients do not respond to schedule their appointments.
Appointment Scheduling:
  • Ensure that the referred services are scheduled and that patients are informed of their appointments. This will involve coordination with the referring office, the office where the patient will be seen, the authorization department, and the patients themselves.
  • Inform the referring provider if a patient is unable to attend or has not accepted an appointment.
Communication and Reporting:
  • Follow up with patients and consulted providers to verify that appointments were attended and services rendered.
  • Communicate promptly with patient care coordinators and authorization specialists at each location.
  • Collaborate with the marketing team and leadership on new initiatives, employing effective communication skills with clients via phone, email, and text.
  • Update referral statuses in the EMR system and generate alerts/reports for stakeholders as necessary.
  • Engage in performance improvement activities to address issues related to referral loop closure, patient satisfaction, and provider satisfaction.
Enhancing Patient Experience:
  • Foster patient satisfaction by demonstrating a commitment to exceptional service, interacting in a friendly and supportive manner.
  • Report potential patient satisfaction concerns to the office and leadership team.
  • Stay updated on the practice's performance regarding patient satisfaction surveys and contribute to overall improvement efforts.
