The Palliative Care Nurse Practitioner plays a crucial role in delivering palliative care to Gentiva patients, ensuring that each treatment plan is tailored to the individual’s needs while demonstrating exceptional clinical expertise and ethical judgment. This role involves diagnosis, treatment, consultation, and follow-up under the guidance of the Palliative Medical Director, as required by state regulations.

Key Responsibilities:

  • Collaborate with the Palliative Care Medical Director and team members to provide comprehensive palliative care, including diagnosis, treatment, and follow-up for patients referred to our services.
  • Assume responsibility for the entire palliative care process, from initial assessment to discharge, focusing on various aspects such as disease progression, functional changes, nutrition, and symptom management.
  • Develop individualized palliative care plans in consultation with patients, families, and the Palliative Care Team.
  • Ensure that patients receive necessary care as identified in their care plans.
  • Participate in on-call services as required, delivering competent and responsive care.
  • Prescribe medications, including controlled substances, within the scope of practice and state regulations. Order and interpret diagnostic tests as needed.
  • Regularly communicate clinical findings to the Palliative Medical Director and reassess care plans to ensure effectiveness.
  • Foster effective working relationships with the supportive care team and maintain open communication regarding patient care.
  • Document clinical findings, treatment plans, and care provided in the patient’s medical record in accordance with established standards.
  • Stay informed about Palliative Medicine Quality Measures and ensure proper documentation and reporting.

Qualifications:

Essential Skills and Knowledge:

  • Strong leadership, interpersonal, and communication skills, both written and verbal.
  • Ability to make independent decisions and demonstrate self-motivation.
  • Understanding of Palliative Medicine principles, Medicare/Medicaid regulations, and interdisciplinary processes.
  • Compliance with continuing education requirements set by Gentiva and licensing boards.
  • Proficiency in utilizing point of care software.

Education and Experience:

  • Master’s degree in Nursing from an accredited institution or equivalent as permitted by state law.
  • A minimum of two years of experience as a Nurse Practitioner.
  • Preferred specialty in Family or Adult Medicine.
  • Experience with Part B billing is advantageous.
  • Ability to work independently in an Advanced Practice Role.

Licenses and Certifications:

  • Current registered nurse license or equivalent as required by state law for Nurse Practitioner licensure.
  • Active Nurse Practitioner certification/license.
  • Collaborative agreement with a supervising physician as required by state law.
  • Part B Provider certification.
  • Federal and state DEA licenses as applicable.
  • ACHPN certification in Hospice and Palliative Care is preferred.
  • Valid driver's license and current automobile insurance.
