Key Responsibilities:- Patient Assessment and Management: Conduct thorough assessments of patients' physical, emotional, and psychosocial needs, and develop individualized care plans to address symptom management, pain control, and overall well-being.
- Collaboration: Work closely with a diverse team, including case workers, chaplains, and hospitalists, to ensure holistic patient care and facilitate communication and coordination among team members.
- Education and Support: Provide disease and treatment education to patients and their families, offer emotional support, counseling, and guidance during difficult times, and assist patients in making informed decisions about their care.
- Advanced Practice Certification: Must be a certified registered nurse practitioner (CRNP) with a focus on palliative care.
- State Licensure: Hold a valid Alabama state license as an advanced practice nurse.
- Experience: Prior experience in palliative care or a related field is preferred.
- Compassion and Empathy: Demonstrate a genuine commitment to providing compassionate care to patients and their families.
