Responsibilities
  • Communicate effectively with residents, families, and healthcare teams to ensure seamless care transitions.
  • Conduct daily resident rounds, identifying and addressing any concerns or issues.
  • Administer medications and treatments as prescribed by physicians, accurately documenting all care provided.
  • Supervise and guide Nursing Assistants in providing direct resident care, promoting a culture of excellence and compassion.
  • Document resident progress notes and provide daily instruction to Nursing Assistants on care-related treatments.
Requirements
  • Current and valid LPN license in Pennsylvania.
