Job Title: Licensed Practical Nurse (LPN) - Home Health

BAYADA Home Health Care is seeking a skilled Licensed Practical Nurse (LPN) to join our team in Cape May Court House, New Jersey. As a home health LPN, you will provide high-quality care to adult and geriatric clients in their homes, working independently and as part of a multidisciplinary team.

Responsibilities:
  • Conduct home visits to assess clients' needs and provide personalized care.
  • Administer medications, perform wound care, and provide treatments as prescribed by physicians.
  • Monitor clients' conditions, reporting changes to the Clinical Manager and Client Services Manager.
  • Follow up with physicians to execute and document their orders.
  • Perform client assessments as necessary and provide case management and coordination.
Qualifications:
  • A current license as a Licensed Practical Nurse in the state of New Jersey.
  • A minimum of one year of recent, verifiable experience as an LPN.
  • Graduation from an accredited and approved nursing program.
  • Prior home care experience is strongly preferred, but not required.
  • Demonstrated ability to read, write, and effectively communicate in English.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
