Scope of Work- Coordinates advanced lung disease and transplantation of all recipients, providing follow-up and care across the continuum from advanced lung disease to pre-transplant, during transplant, and post-transplant.
- Collaborates with members of the healthcare team to enhance patient care by developing, supporting, and educating nursing staff.
- Makes clinical judgments based on current research, concepts, principles, theories, and outcomes to solve complex problems.
- Facilitates advanced nursing care for patients before and after transplantation along the continuum of outpatient to inpatient as required.
- Coordinates evaluation process for pre-transplant patients and living donors to ensure effective use of hospital and/or clinical resources.
- Assesses post-transplant and post-donation organ function and general health status, determining the need for additional assessment or intervention.
- Participates in the development and implementation of protocols and standards of care for the transplant patient in collaboration with the multidisciplinary team.
- Provides on-call coverage, coordinating activities prior to transplant, the transplantation process, and communication between physicians, local organ procurement organization, patients/families, and the medical team involved.
- Completes necessary documentation according to UNOS guidelines, including listing, status changes, and post-transplant/post-donation follow-up information.
- Communicates with multidisciplinary team members and patients and families in a timely manner regarding patient care/issues and develops discharge plans for patients pre-transplant or post-transplant, including referrals for necessary discharge care.
- Educates patients, caregivers, and hospital staff along the continuum by developing patient education materials or identifying external resources to assist understanding of processes and procedures surrounding pre-, peri-, and post-transplant issues.

- Required: Bachelor's Degree in Nursing, 5 years of relevant nursing experience, MI Registered Nurse (RN) license, Basic Life Support (BLS) certification, and Advanced Cardiovascular Life Support (ACLS) certification.
