Community Support Specialist

Job Summary:
The Direct Support Professional (DSP) Community is responsible for implementing individualized habilitation plans, attending to personal care needs, and overseeing the health, safety, and well-being of individuals with disabilities.
Key Responsibilities:
- Implement individualized habilitation plans to support individuals with disabilities
- Attend to personal care needs, including bathing, dressing, and grooming
- Oversee the health, safety, and well-being of individuals with disabilities
- Participate in agency activities and transport individuals to appointments and community activities
- Maintain accurate documentation of individual progress and services provided
Requirements:
- High school diploma or equivalent required
- Valid NYS driver's license required
- No prior experience necessary - paid training provided
