Job Summary

CharterCARE Health Partners seeks a skilled Referral Coordinator to ensure the quality of the referral process is upheld to departmental and system standards. The ideal candidate will develop and maintain collaborative relationships with providers and outpatient practices, ensuring accurate and timely referrals.

Key Responsibilities
  • Review incoming referrals for accuracy and completeness, ensuring patient demographics and provider requests are accurately reflected.
  • Communicate with patients to gather additional information and obtain necessary authorizations.
  • Monitor referral quality and timeliness, ensuring targets are met and quality control is upheld.
  • Develop and maintain relationships with healthcare providers and outpatient practices.
  • Provide exceptional customer service to patients and their families, ensuring a compassionate and competent experience.
Requirements
  • High School Diploma or equivalent required; advanced medical degree/certification preferred.
  • Bilingual Portuguese/Spanish required/strongly preferred.
  • Four plus years of relevant work experience in a medical/physician's office required.
  • Previous knowledge and experience processing online/phone authorizations required.
  • Previous coding/billing experience preferred.
Preferred Qualifications
  • Knowledge of basic software applications, including Electronic Health Record management.
  • Experience working with Athena and knowledge of Meditech and Infina.
  • Strong interpersonal and customer relations skills.
  • Ability to lead multiple staff/disciplines and handle multiple priorities with minimal supervision.
  • Ability to comprehend and follow written and verbal instructions, and maintain confidentiality of patient and employee information.
