The Salvation Army is seeking a skilled Case Management Specialist to join our team. As a key member of our social services department, you will be responsible for providing comprehensive, long-term case management services to clients participating in our life management program.
Key Responsibilities
- Conduct thorough assessments of client needs and develop individualized program plans to achieve their goals.
- Provide ongoing support and guidance to clients, including mentoring and counseling sessions.
- Collaborate with community agencies and resources to ensure clients receive the necessary services to achieve stability and self-sufficiency.
- Monitor client progress and adjust program plans as needed to ensure successful outcomes.
- Maintain accurate and complete case notes and client records.
Requirements
- Bachelor's degree in Social Work, Behavioral Science, or a related field.
- Three years of experience providing direct case management social services, including accessing client needs and developing individualized program plans.
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, community agencies, and internal stakeholders.
- Ability to work in a fast-paced environment and prioritize multiple cases simultaneously.
- Physical ability to sit for extended periods and perform light physical tasks, such as walking and standing.
