Key Responsibilities

  • Conduct thorough assessments of client needs and develop individualized program plans to achieve their goals.
  • Provide ongoing support and guidance to clients, including mentoring and counseling sessions.

  • Monitor client progress and adjust program plans as needed to ensure successful outcomes.
  • Maintain accurate and complete case notes and client records.

Requirements

  • Bachelor's degree in Social Work, Behavioral Science, or a related field.
  • Three years of experience providing direct case management social services, including accessing client needs and developing individualized program plans.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, community agencies, and internal stakeholders.
  • Ability to work in a fast-paced environment and prioritize multiple cases simultaneously.
  • Physical ability to sit for extended periods and perform light physical tasks, such as walking and standing.
