Job Title: Case Manager

Avita Community Partners is seeking a skilled Case Manager to join our team. As a Case Manager, you will provide a broad range of paraprofessional behavioral health and social services to clients and their families.

Key Responsibilities:
  • Organize and facilitate groups under supervision.
  • Participate in treatment team planning and perform crisis intervention.
  • Teach daily living skills to clients and monitor/document progress toward treatment goals.
  • Assist clients in locating and utilizing community resources, including legal, medical, financial assistance, and other referral services.
  • Counsel clients living in group homes and halfway houses, and assist in pre-release and release planning.
  • Implement and organize the delivery of specific social services within the community.
  • Implement life skills workshops and programs in behavior management, youth services, community and social services.
  • Maintain contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on clients overall progress.
  • Monitor client's progress towards treatment goals.
  • Provide short-term case management and referral services to clients with emergency situations.
Requirements:
  • Completion of a Bachelor's Degree in Social Work, Psychology, Mental Health, Counseling or related field.
  • One year of experience working in a community mental health setting providing direct services (Level 3 @ $34,840).
  • Must not have six (6) or more points against their driver's license or a suspended or revoked driver's license within the last five (5) years.
