Certification Requirement

An Immunization Certification from an accredited organization (e.g., APhA) is mandatory.

Key Responsibilities

1. Assess physician medication orders for suitability regarding drug selection, dosage, potential interactions, administration routes, and stability. Provide recommendations for therapy modifications to physicians and facility personnel as necessary.

2. Conduct monthly reviews of the medication regimens and clinical indicators for each assigned resident, reporting any discrepancies to the Director of Nursing, Administrator, and attending physicians with a detailed written report of findings and suggestions.

3. Document all interactions with physicians, nursing staff, other healthcare team members, and patients/residents along with their families.

4. Stay informed about pharmacy laws and federal and state regulations related to long-term care.

5. Collaborate effectively as a team member.

6. Maintain up-to-date knowledge of pharmaceutical care practices for residents in long-term care settings.

Qualifications

- Bachelor's degree from an accredited School of Pharmacy or a Doctor of Pharmacy degree.

- A current pharmacy license is required; dual licensure is preferred.

- A minimum of one year of experience in acute care, long-term care, or home health is preferred.

- Experience as a Consultant or Clinical Pharmacist is advantageous.

- IV clinical experience is preferred.

- Availability to work weekends or outside of standard business hours as needed.

- Ability to lift patient charts repeatedly.

- Capability to stand for extended periods.

- Strong customer service skills.

- Ability to comprehend and adhere to state and federal regulations for long-term care.

- Understanding of formulary management.
