Job Overview
The Caregiver Nurse Specialist will be responsible for providing exceptional care to our residents, ensuring their overall well-being and promoting a safe and healthy living environment. This role requires strong communication skills, attention to detail, and the ability to work effectively as part of a team.
Duties and Responsibilities
- Identify ongoing needs and services for residents, promoting the highest quality resident services.
- Ensure proper follow-through and participate in evaluations and documentation for residents with changes in healthcare services or status.
- Assist the Resident Care Director in conducting resident evaluations and gathering information at move-in, 30 days post-move-in, semi-annually, and with any change in status.
- Conduct monthly wellness visits for all residents and monitor those who have experienced falls, accidents, or incidents, documenting and reporting as required.
- Maintain effective communication with residents' families and responsible parties to ensure medical needs are met.
- Contact attending physicians when necessary and/or upon family request.
- Obtain weights and vital signs monthly for each resident and assist in maintaining wellness files according to Sunrise policies and federal/state regulations.
- Provide clinical support and assistance to community team members as needed, adhering to infection control practices and universal precautions.
- Regularly monitor each resident's medication and treatment profile to ensure accurate administration and documentation.
Requirements and Qualifications
- Graduate of an approved college/school of nursing with a current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum one year experience in home health, assisted living, hospital, or long-term care environments, with knowledge of nursing practices, techniques, and methods applied to health and wellness resident care and federal, state, and local regulations.
- Excellent communication and interpersonal skills, with the ability to teach and guide team members to provide quality clinical care.
- Competent in organizational and time management skills, with good judgment, problem-solving, and decision-making abilities.
Compensation and Benefits
We offer competitive compensation based on geographic location, skills, experience, qualifications, and certifications/licenses. The estimated salary range is $27.00 - $31.05 per hour. Overtime eligibility applies. Additionally, we offer a comprehensive benefits package, including medical, dental, vision, life, and disability plans, retirement savings plans, employee assistant program/discount program, paid time off (PTO), sick time, holiday pay, daily pay, and tuition reimbursement.
