We are seeking a skilled Registered Nurse to join our Case Management team at Ascension St. Vincent's St. Clair. As a key member of our interdisciplinary team, you will play a vital role in coordinating patient care from admission to discharge.

Responsibilities
  • Develop and implement comprehensive care plans that integrate patient preferences and values.
  • Monitor patient care through regular assessments and evaluations.
  • Advocate for resources and remove barriers to ensure effective care delivery.
  • Maintain open communication with supervisors and healthcare providers to ensure seamless care implementation.
  • Act as a resource for accurate medical record documentation and treatment options.
Requirements
  • Licensure: Licensed Associate Professional Registered Nurse credentialed from the Alabama Board of Nursing.
  • Education: Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
