We are now seeking a skilled Medical Social Worker II to join our team

About the Job:
As a Medical Social Worker II, you will play a vital role in providing compassionate and quality care to our patients and their families. Under the direction of the Social Work Director or Operations Manager of Home Care Services, you will provide services aimed at enhancing functioning and addressing psychosocial problems that interfere with recovery from illness or prevent a healthy lifestyle.

Your primary responsibilities will include:
  1. Utilizing social work skills to identify high-risk patients with psychosocial problems.
  2. Establishing rapport and effective relationships with patients and families to facilitate timely work.
  3. Evaluating data, assessment, and identifying strengths, weaknesses, and problem areas related to treatment plans.
  4. Developing and implementing interventions to enable patient/family coping with problems, including grief, adjustment to lifestyle changes, financial burdens, and abandonment feelings.

Requirements:
To be successful in this role, you will need:
  1. A Master's degree in Social Work (MSW) and two-three years of experience in a healthcare setting.
  2. LCSW eligibility and knowledge of entitlement and pre-admission screening programs.
  3. Excellent written and verbal communication skills, as well as empathy, sensitivity, patience, and stress tolerance.

About the Salary:
The estimated salary for this position is $70,000-$110,000 per year, depending on your level of experience and qualifications.

Work Environment:
As a Medical Social Worker II, you will work in various settings throughout the hospital, including nursing floors, patient rooms, Emergency Department, clinics, and occasionally in patient homes. You will interact with patients, families, and other healthcare providers, often in high-pressure situations.

This role requires excellent time management, prioritization, and organizational skills to ensure efficient completion of tasks and adherence to hospital policies and regulatory requirements.
