About the Role:

Bristol Home Care & Hospice Agency Inc. is seeking a highly skilled Medical Social Worker to join our team. This role offers an exciting opportunity to make a meaningful impact on patients' lives by providing expert social work services and supporting their overall well-being.

As a key member of our healthcare team, you will be responsible for conducting thorough patient assessments, developing individualized care plans, and collaborating with interdisciplinary teams to ensure seamless care transitions.

Salary: $75,000 - $90,000 per year, depending on experience.

Main Responsibilities:

  • Conduct comprehensive patient assessments to identify emotional, social, and environmental challenges.
  • Develop and implement individualized care plans that address patients' unique needs.
  • Provide counseling and guidance to patients and their families to address complex medical situations.
  • Collaborate with healthcare teams to ensure coordinated care and effective communication.
  • Document patient interactions and care plans accurately and efficiently.

Qualifications:

  • Master's degree in Social Work (MSW) from an accredited institution.
  • Experience working in healthcare settings, preferably in home care or hospice environments.
  • Strong knowledge of psychosocial assessments, treatment plans, and crisis intervention techniques.
  • Excellent communication and interpersonal skills.
